Your patients are already on WhatsApp
Ask any UK aesthetics clinic owner where enquiries come from now, and WhatsApp is near the top of the list. Patients like it because it's quick and private. They can send a photo, ask about prices and book without making a phone call.
The catch is that they expect a WhatsApp reply in minutes, not hours. If you're answering from your own phone between treatments, that's hard to keep up.
That's where the WhatsApp Business API comes in. It's the version of WhatsApp that lets you connect an AI assistant, your booking system and automatic reminders to your clinic's number. This guide explains, in plain English, what it is, what it costs in the UK, the rules you need to follow, and how to get set up.
WhatsApp Business app vs the WhatsApp Business API
Most clinics start with the free WhatsApp Business app on a phone. It's great to begin with. The API (Meta calls it the WhatsApp Business Platform) is what you move to when the app can't keep up.
Cost: the app is free. The API is pay per message (see costs below).
Who replies: with the app, you or your team reply by hand. With the API, an AI assistant, your team, or both.
Devices: the app works on your phone plus a few linked devices. The API lets any number of team members share one number.
Automation: the app has an away message, greeting and quick replies. The API adds AI replies, bookings into your diary, reminders and follow-ups.
Booking system: the app can't connect to it. The API can, through a provider.
Best for: the app suits low volumes where the owner answers everything. The API suits busy clinics, out-of-hours enquiries and growth.
In short: the app is a phone. The API is the plumbing that lets software, including an AI assistant, answer your clinic's WhatsApp for you.
What the API lets an aesthetics clinic do
Once your number is on the API, you can connect tools that work on WhatsApp around the clock:
AI replies in seconds. Answers prices, downtime, aftercare and "do you have anything Saturday?" in your clinic's tone, at 11pm or mid-treatment.
Bookings straight into your diary. The assistant offers real appointment slots and books the consultation.
Appointment reminders. Automatic reminders the day before cut no-shows, and patients can reply to confirm or rearrange.
Aftercare and review requests. A check-in message after treatment, then a friendly request for a Google review.
Rebooking nudges. A reminder when it's time for a top-up, so patients come back to you rather than shopping around.
A shared inbox. Your whole team sees every conversation, so nothing sits unread on one person's phone.
Anything clinical, such as a patient worried about swelling, should be handed straight to you. A good setup is built that way from day one.

What the WhatsApp Business API costs in the UK
There are two parts to the cost: Meta's message fees and your provider's fee for the software that runs on top.
Meta's fees changed on 1 October 2026. Until then, replying to a patient who messaged you first was free. From 1 October, Meta charges per message for those replies (service messages) and for utility messages sent inside the 24-hour reply window (Meta). Patients messaging you is still free.
Approximate UK rates per message (whautomate):
Service (replying to a patient's question): about 1.6p
Utility (appointment reminder, booking confirmation): about 1.6p
Marketing (offer, newsletter, "time for your top-up"): about 4.6p
Meta sets these rates and can change them, and some accounts are billed in US dollars, so check your provider's current rate card.
Example: a clinic handling 1,700 replies and reminders a month, plus 100 marketing messages, would pay Meta roughly £32 a month. Provider fees for the software, inbox and AI assistant are usually on top of that.
Put against one extra booking a month, the message fees are small. The bigger question is whether the setup actually turns conversations into appointments.
The rules every clinic needs to know
The API has rules the free app doesn't. A good provider will set these up for you, but it helps to understand them:
The 24-hour window. When a patient messages you, a 24-hour window opens in which you can reply freely in normal conversation (Meta).
Templates outside the window. To message a patient first, or after 24 hours, you must use a message template that Meta has approved, such as an appointment reminder.
Opt-in. Only message patients who have agreed to hear from you on WhatsApp. Keep a record of it, and make it easy to opt out.
UK GDPR. Patients may share health details or photos. Make sure your provider explains where data is stored and signs a data processing agreement with you.
Prescription-only medicines. Botox and other anti-wrinkle injections can't be advertised to the public in the UK, and that includes WhatsApp marketing messages. Keep promotions to consultations and non-prescription treatments.
Breaking Meta's rules can get your number restricted, so this is worth getting right from the start.
How to get set up
You don't connect to the API directly. You go through a provider (Meta calls them Business Solution Providers or Tech Providers), who handles the technical side. The usual steps:
Set up a Meta business portfolio for your clinic, if you don't already have one for Facebook or Instagram.
Choose a provider and connect your clinic's WhatsApp number.
Verify your business and display name with Meta.
Get your message templates approved, such as reminders and booking confirmations.
Train the AI assistant on your treatments, prices, policies and tone of voice, then test it properly before going live.
Questions to ask any provider:
Can I keep my existing clinic number, and can I still use the WhatsApp Business app on it?
Does it connect to my booking system (Pabau, Phorest, Fresha, Timely or other)?
What is the total monthly cost, including Meta's message fees?
Where is patient data stored, and will you sign a data processing agreement?
How are clinical or urgent messages passed to me?
Who updates the assistant when my prices or treatments change?
WhatsApp Business API FAQs
Is the WhatsApp Business API free in the UK?
No. Since 1 October 2026, Meta charges per message for most messages a business sends, including replies to patients. Messages patients send to you are free. Expect a few pence per message, plus your provider's fee.
Do I need the API, or is the free app enough?
The free app is enough if you're answering a handful of messages a day yourself. Move to the API when enquiries go unanswered for hours, when your team needs to share one number, or when you want an AI assistant to reply and book for you.
Can an AI assistant reply to patients on WhatsApp?
Yes, through the API. It can answer common questions and book consultations. It should never give medical advice, and clinical questions should go straight to you.
Can I send appointment reminders on WhatsApp?
Yes. Reminders are sent as approved utility templates and are one of the most useful things the API does. Patients can reply to confirm or rearrange.
Can I send promotions on WhatsApp?
Yes, to patients who have opted in, using approved marketing templates. Keep it relevant and occasional, and don't promote prescription-only treatments like Botox.
